STUDENT FORMS FOR STUDY ABROAD
Below is a list of forms (in addition to the ISIC card application, which can be obtained from the Education Abroad office in Gordy Hall) that all students must complete and submit to the Global Studies Coordinators in order to participate in and receive credit for the "Global Studies" field course. The forms must be completed and submitted no later than the fifth week of the quarter preceding the field course, and should be submitted as soon as possible before then.
The forms are in Word for Windows '95 format, and are linked to the name of the document below. The document can be saved as a blank form to your computer and completed and printed out later; or completed when you open it up, then printed out (and signed, if a signature is required). While you must submit a hard copy of the completed forms, it is appropriate for you to keep the completed computer document for your records (to make it easier for you to make any changes where needed). If you run into problems interpreting a particular item requesting information or a question, contact one of the coordinators.
Study Abroad student forms
Students must remember that an International Student Identification Card (ISIC) application must also be obtained from the Education Abroad office in 107 Gordy Hall, completed and submitted to that with $22, in order to get the required ISIC card. The card provides the minimal insurance coverage demanded by the Study Abroad program and Ohio University.
